How to read the Measure Status Legend

The list of patient visits on the Visits page displays a status icon for each measure. These icons allow you to view a measure's performance at a glance.

Incomplete: Represented by a red half-filled circle, Incomplete means a measure is missing information. If you’re using data integration, this typically means the measure's denominator (eligibility) criteria is met, but the numerator (performance) isn't complete. If you’re using manual entry, it means you didn't answer all questions for a measure. 

Ineligible: Represented by an empty black circle with a slash through it, Ineligible means a visit didn't meet a measure's denominator (eligibility) criteria. This could be for several reasons — lack of diagnosis code, lack of encounter code, outside age range, etc.

Ineligible visits don't contribute to a measure's performance calculations, nor do they affect the reporting rate requirement. If you believe a visit is eligible but is marked ineligible, review the measure description.

Excluded: Represented by an empty black circle, Excluded means a patient meets the measure's eligibility criteria but has a documented exclusion. Exclusions only apply to some measures and can be found in the measure description. Exclusions don't contribute to or reduce performance. 

Not met: Represented by a solid burnt orange circle, Not met means the patient is denominator-eligible for the measure, but the performance wasn't met or recorded, and the patient doesn't have an exclusion. Not met visits reduce the performance rate of the measure. For inverse measures, however, Not met is the desired outcome.

A visit can have a status of Not met even if the performance or numerator code isn't recorded. For example, you may see a status of Not met for Measure 130: Documentation of Current Medications in the Medical Record if MIPSpro doesn't receive the Met performance code. If you believe any visits that have a Not Met status should be Met, review and update the visit as long as there is documentation to confirm the quality action behind the measure was completed.

Met: Represented by a solid green circle with a white check mark, Met means the visit is denominator-eligible for the measure and met the performance requirements. For inverse measures, however, Met is an undesired outcome.

Not applicable: Represented by N/A, Not applicable means the measure questions haven't been activated or answered by any codes. It may also mean a visit is ineligible because it doesn't meet eligibility criteria or insufficient information has been provided to determine eligibility criteria.

Some measures contain multiple denominator criteria, called strata, within the same measure. A multiple stratum measure, such as Measure 226, will appear as several columns on the Visits page, (#226, #226 S2, and #226 S3).

Some measures’ strata contain mutually exclusive requirements (such as ages 18-64 for stratum 1, ages 65-plus for stratum 2), so it isn't cause for concern if some strata lack performance.
