Using the Patient Record in MIPSpro

The Patient Record houses several tabs — Demographics, Measures, and Notes.

Demographics

This tab displays all previously added patient demographic information. It can be updated at any time before submission.

Patient Identifier, Date of Birth, and Gender are required fields.

Measures

This tab displays information about all visits associated with the patient ID. It also includes visits entered during past reporting years.  

Some highlights of the Measures tab include: 

1. Associated NPI(s). This section displays all NPIs associated with the patient. MIPSpro provides the option to add an associated NPI to each patient visit.

2. Visit Summary. This section displays a summary of all visits associated with the patient ID.

3. Visit Dates. This section includes Visit Dates and Associated Codes. The codes are retained only for data integration users or those entering codes when adding visits.

4. Measure Summary. This section provides details about each measure, including the current status, included instances, and overall performance.

Notes

This tab allows you to enter notes about a patient. Notes are for your records and won't be reviewed by MIPSpro or used to determine your performance. They also aren't sent to CMS. 

Click Add Note to open the Notes modal.

  • Type your note in the modal and click SAVE to keep it.
  • Click CANCEL to return to the Patient Record without saving the note.

Best practices

  • We recommend that all patient identifiers be de-identified. Acceptable patient identifiers include:
    • Patient chart number
    • Medical Record Number (MRN)
    • Any unique combination of letters and numbers
  • Use the Included Instances column of the Measure Summary section to understand each instance that contributes to the measure's performance. Click the date of service to go to the associated visit. You can then view, edit, and update all patient and visit information.
